SMS Terms & Conditions

These Terms & Conditions govern the SMS text messaging program operated by Companion Health PLLC (“Companion Health,” “we,” “us,” or “our”). By providing your mobile number and opting in, you agree to the terms below.
1. Program description
When you opt in to the Companion Health PLLC SMS program, we may send you text messages related to your care and your relationship with our practice, including:
- Appointment reminders, confirmations, and scheduling updates
- Care coordination and follow-up messages from your care team
- Billing, forms, and account notifications
- Responses to questions you send us and customer support follow-ups
- Occasional practice news and health program announcements
The purpose of these messages is to support your care and keep you informed about the services and updates relevant to you.
2. Opting out
You can cancel the SMS service at any time. Simply text STOP to the number from which you received our messages. After you send STOP, we will send you a confirmation message and then stop sending SMS messages. If you want to rejoin, sign up as you did the first time and we will resume sending messages to you.
3. Getting help

4. Message frequency
Message frequency varies. You may receive recurring messages based on your care and interactions with us, up to approximately 4–6 messages per month. Transactional messages, such as appointment reminders, are sent as needed.
5. Costs
Message and data rates may apply.
These charges are billed by and payable to your mobile service provider. Please contact your wireless carrier for details about your messaging plan. Companion Health PLLC does not charge for the SMS service itself.
6. Carriers
Carriers are not liable for delayed or undelivered messages.
7. Privacy
We respect your privacy. We will not share or sell your mobile opt-in information or phone number for the purpose of third-party marketing. Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes. For details on how we handle your information, see our Privacy Policy.
8. Changes to these terms
We may update these Terms & Conditions from time to time. Changes are effective when posted to this page. Your continued participation in the SMS program after any change constitutes acceptance of the updated terms.
